ACORD 126 — Commercial General Liability Section
Attach to ACORD 125 when applying for or quoting commercial general liability (CGL) coverage. Captures operations, premises, products/completed operations, and exposure data.
ACORD 127 — Business Auto Section
Attach to ACORD 125 when applying for commercial auto coverage. Lists scheduled vehicles, hired/non-owned auto exposure, and driver information.
ACORD 130 — Workers Compensation Application
Apply for workers compensation and employers liability coverage. Captures payroll by class code, experience modification, and state-specific information.
ACORD 131 — Umbrella / Excess Liability Section
Attach to ACORD 125 when applying for commercial umbrella or excess liability coverage. Lists underlying policies and limits that the umbrella sits above.

Supplement to the commercial application for contracting risks. Captures subcontractor usage, job types, safety programs, and equipment details.
ACORD 137 — Commercial Property Section
Attach to ACORD 125 when applying for commercial property coverage. Captures building details, values, construction type, protection class, and business income information.
ACORD 140 — Property Section (Building)
Detailed building-specific supplement used alongside ACORD 137. Captures construction details, square footage, renovations, and building-level values for each scheduled location.
ACORD 141 — Property Section (Schedule)
Schedule of locations and values for commercial property policies with multiple buildings or premises. Pairs with ACORD 137 and 140.

ACORD 1 — Property Loss Notice
Report a property loss (fire, theft, water damage, wind, etc.) to the carrier. Used for both personal and commercial property claims as the first notice of loss (FNOL).
ACORD 2 — Auto Loss Notice
Report an automobile loss (collision, comprehensive, liability) to the carrier. Captures accident details, vehicle damage, and involved parties.
ACORD 3 — General Liability Loss Notice
Report a general liability occurrence (bodily injury, property damage caused by insured, personal/advertising injury) to the carrier.
ACORD 4 — Workers Compensation Loss Notice (First Report)
Report a workplace injury to the workers compensation carrier. Often required by state law within specific timeframes (24-72 hours in most states).

ACORD 35 — Cancellation Request / Policy Release
Request cancellation of a policy on behalf of the insured, or release a policy to another agency. Used for both mid-term cancellations and agency-of-record changes.
ACORD 36 — Change Request / Endorsement Request
Request a mid-term policy change or endorsement — add a vehicle, change an address, add additional insured, increase limits, or modify coverages.
ACORD 37 — Audit Request
Request a premium audit or provide audit information to the carrier, typically for workers compensation or general liability policies where premium is based on payroll or receipts.
ACORD 38 — Agency Record
Document basic agency information for carrier appointments, agency agreements, or internal records. Captures agency details, principal/owner info, and E&O coverage.

ACORD 45 — Additional Interest Schedule
Add or manage additional interests (additional insureds, loss payees, mortgagees, certificate holders) on a policy when the list exceeds what fits on the main form.
ACORD 60 — Request for Certificate Holders
Collect certificate holder information from the insured so certificates can be issued. Send this to clients who need to provide a list of parties requiring COIs.
ACORD 75 — Insurance Binder
Issue temporary proof of insurance before the formal policy is issued by the carrier. Binders are legally binding and provide coverage during the interim period.
ACORD 101 — Additional Remarks Schedule
Attach to any ACORD form when you need more space for additional information, special conditions, or remarks that don't fit in the standard form fields.

What Are ACORD Forms?
ACORD (Association for Cooperative Operations Research and Development) forms are standardized documents used across the insurance industry in the United States and internationally. They provide a uniform format for exchanging information between agents, brokers, carriers, and insureds.
ACORD forms streamline everything from new business applications to certificates of insurance to loss notices. Because every carrier and agency uses the same form formats, information flows more efficiently and errors are reduced. There are hundreds of ACORD forms, but most agencies regularly use only 20 to 30 of them.
ACORD forms are maintained and updated by ACORD, a nonprofit standards organization. Forms are periodically revised — always check that you are using the most current edition accepted by your carrier partners.

ACORD Form Tips for New CSRs
Always use the current edition
Carriers may reject outdated form versions. Check the edition date in the bottom-left corner of every ACORD form before submitting.
Certificates are evidence only
The ACORD 25 (and 27/28) cannot alter, extend, or modify policy coverage. If a certificate holder asks for specific language, that requires an endorsement on the actual policy.
The 125 never goes alone
The Commercial Insurance Application (ACORD 125) is always accompanied by line-specific supplements — 126 for GL, 127 for auto, 130 for WC, 137 for property, etc.
Specify effective dates on changes
When submitting an ACORD 36 (Change Request), always specify the exact date the change should take effect. Do not leave it blank or assume "today."
File loss notices immediately
ACORD 1, 2, 3, and 4 should be submitted as soon as you learn of a loss. Late reporting can jeopardize coverage and creates E&O exposure for your agency.
Use ACORD 101 for overflow
When any form runs out of space, attach an ACORD 101 (Additional Remarks Schedule) rather than scribbling in margins or attaching loose notes.
