Understanding ISO Insurance Forms
ISO (Insurance Services Office) publishes standardized insurance policy forms, endorsements, and rules used by the majority of property and casualty insurers in the United States. Each form is identified by a prefix indicating the line of business and a unique number.
Insurance agents need to understand form numbers to verify coverage, compare policies across carriers, identify endorsements, and ensure the correct coverage forms are in place. Many carriers use ISO forms as-is or with proprietary modifications.
Form Number Structure
- Prefix (2 letters) - Indicates line of business (CG = General Liability, CP = Property, etc.)
- First two digits - Form category (00 = coverage forms, 20 = additional insured, 21 = exclusions, 24 = other endorsements, 25 = limit changes)
- Last two digits - Specific form within that category
- Edition date - Month and year of the form version (e.g., 04/13 = April 2013)
